Adult AuDHD Assessment: Report Template

Description

Writing a comprehensive assessment report takes time, especially when you want it to be clinically clear, neurodiversity-affirming and meaningful to the person reading it. I’ve created this template to make that process a little easier. Drawing on my clinical experience, it brings together an editable report structure, suggested wording and practical guidance to help you communicate your findings with care and confidence. It’s designed for psychologists completing adult Autism and ADHD combined assessments, with sections you can adapt to your assessment process and each client’s experiences.

What’s included?

The template includes guidance and editable sections covering:

  • Clinical interviews: Prompts for presenting MIGDAS-2 and DIVA-5 findings.
  • Standardised measures: Explanations, blank results tables and interpretation guidance for measures including the SRS-2, CATI, RAADS-R, SP-2, BRIEF-A, ASRS and CAARS-2.
  • Differential considerations: A framework for considering alternative and co-occurring explanations, including relevant PAI findings.
  • Functional capacity: An ABAS-3 section and prompts for describing everyday strengths, participation and support needs.
  • Findings and diagnosis: Guidance for explaining diagnostic reasoning, presentations, support levels and relevant specifiers.
  • Clinical formulation: Prompts for bringing the findings together into an individualised understanding of your client.
  • Recommendations: Examples and guidance for developing practical supports linked to the client’s goals and assessment findings.

Affirming language, clear findings

A report can recognise strengths and respect neurodivergent experiences while clearly documenting distress, barriers and support needs. This template helps you maintain that balance. It encourages you to explain what results mean in everyday life, acknowledge uncertainty and connect your conclusions with the evidence gathered throughout the assessment. The aim is a report that feels personal and understandable to your client while providing useful clinical information to other professionals.

Make it work for you and your client

This is a flexible guide. Add, remove or amend sections to suit your clinical approach, the measures you administer and the person you are assessing. You’ll find prompts to help you include client-specific examples, distinguish reported experiences from clinical observations, and recognise the effort and support behind everyday achievements.

A few things to know

This template supports professional judgement and should be used within your training and scope of practice. It does not explore assessment instruments in depth, test items, scoring manuals or licences to administer the measures discussed. All wording, scores, interpretations and recommendations need to be reviewed and personalised before a report is finalised.

Purchase is for use by one clinician, who may adapt the template for their own client reports. Please do not share, forward, upload, resell or redistribute the reusable template, including edited versions. Completed client reports can, of course, be shared appropriately as part of clinical care.

A lot of time and care has gone into creating this resource. Thank you for supporting that work – I hope it makes your report writing feel more manageable.
